O2 Sensor Signal Biased/Stuck Rich Bank 2 Sensor 1
The Powertrain Control Module (PCM) detected that the upstream oxygen sensor (Sensor 1) on Bank 2 is continuously reading a rich exhaust condition. This means the sensor's voltage signal is stuck high and is not fluctuating normally as the PCM attempts to adjust the air-fuel ratio.
While the vehicle will usually still drive, running continuously rich can eventually damage the catalytic converter and foul spark plugs. It should be diagnosed and repaired soon to prevent more expensive emissions system failures.

Induce a temporary lean condition
Look for: Create a small vacuum leak while monitoring the O2 sensor voltage. If the voltage drops in response to the extra air, the sensor is working and the engine is actually running rich; if it stays high, the sensor or wiring is likely faulty.
Inspect the O2 sensor wiring and connector
Look for: Visually check the Bank 2 Sensor 1 harness for melted wires, corrosion, or loose pins. Look for continuity issues or shorts to voltage that would artificially inflate the sensor signal.
Perform a fuel pressure test

Perform a fuel injector leak down test
